Transitions Manager — Young Adults Social Care Leader

Jobtailor is seeking a Lead for the Young Adults Service within Adult Social Care in Preston. You will provide leadership, professional oversight, and operational management to Social Workers and their teams to deliver high-quality safeguarding and support planning.

You will work closely with education, health, housing and community partners to coordinate services for young adults with a range of needs, including learning disabilities, autism, physical disabilities and mental health challenges.
