About the Role

Our client requires a dedicated Social Care Assessor to join their team in Cardiff , contributing to the provision of essential social support within the community. In this role, you will conduct thorough assessments of individuals' needs for social care services, ensuring that appropriate support plans are developed and implemented. You will play a crucial part in empowering individuals to maintain their independence and quality of life. This is an important role within the Welsh social care system, offering the chance to work with a diverse client group and collaborate with various agencies to deliver impactful support.

Key Responsibilities

• Undertake comprehensive assessments of individuals' needs for social care services, adhering to national and local eligibility criteria.
• Develop, review, and monitor personalised support plans in collaboration with individuals, families, and carers.
• Make recommendations for services and coordinate provision with internal and external providers.
• Ensure all assessments and support plans are accurately recorded and maintained within the case management system.
• Provide information, advice, and support to individuals and their families regarding available services and entitlements.
• Liaise effectively with health professionals, housing services, and other agencies to ensure seamless care pathways.

Requirements

• Relevant qualification in Social Work, Health, or a related field (e.g., Diploma in Social Work, NVQ Level 4/5 in Health and Social Care).
• Previous experience in conducting needs assessments within a social care setting.
• Thorough knowledge of relevant legislation, policies, and procedures governing social care in Wales .
• Excellent analytical, report-writing, and decision-making skills.
• Ability to manage a caseload effectively and prioritise workload.
• Strong interpersonal and communication skills, with the ability to engage sensitively with clients.
