About the Role
Our client seeks a proactive and detail-oriented Social Care Assessor to join their vital services in Coventry . This role involves evaluating individuals' needs for social care support, determining eligibility, and facilitating access to appropriate services. Working in a hybrid capacity, you will balance essential in-person assessments within the community with remote administrative and planning tasks. Key Responsibilities
- Conduct thorough assessments of individuals' social care needs in their homes or chosen settings.
- Determine eligibility for services based on local authority guidelines and national policies.
- Develop, implement, and review care plans in collaboration with clients, families, and other professionals.
- Liaise with a range of service providers to arrange and coordinate care packages.
- Maintain accurate and up-to-date case records using the organization's IT systems.
- Provide information and advice to clients and their families regarding available support options.
Requirements
- Experience in a similar social care or assessment role.
- Knowledge of relevant legislation, policies, and procedures in social care.
- Excellent assessment, report-writing, and communication skills.
- Proficiency in using IT systems for case management and record-keeping.
- Ability to manage a caseload effectively and meet performance targets.
