About the Role

Our client seeks a dedicated and insightful Social Care Assessor to join their fully remote operations team. In this critical role, you will conduct comprehensive assessments of individuals' needs within the community, evaluating their circumstances and recommending appropriate support services. You will play a vital part in ensuring vulnerable individuals receive the assistance they require to live more independent and fulfilling lives. Key Responsibilities

• Conduct thorough needs assessments for individuals seeking social care support, adhering to national and local guidelines.
• Develop personalized support plans in collaboration with clients, their families, and other relevant agencies.
• Evaluate the effectiveness of existing care packages and recommend adjustments as needed.
• Maintain accurate and detailed case records using designated IT systems and databases.
• Provide information and advice to clients and their families regarding available social care services and resources.
• Liaise with internal and external stakeholders, including support providers and local authorities, to facilitate seamless service delivery.

Requirements

• Experience in conducting assessments within a social care or health-related setting.
• Sound knowledge of relevant legislation, policies, and best practices in social care.
• Excellent interviewing, communication, and report-writing skills.
• Ability to work independently, manage a caseload effectively, and meet deadlines remotely.
• Proficiency in using digital assessment tools and case management software.
• A qualification such as NVQ Level 3 or 4 in Health and Social Care, or equivalent.
