Social Care Officer

About the Role

Our client is looking for a motivated Social Care Officer to join their team in Bristol . In this role, you will play a crucial part in assessing the needs of individuals and families, developing support plans, and coordinating care services to ensure their well-being and independence. You will work closely with clients, their families, and a network of external agencies to deliver effective and person-centered support. Key Responsibilities

• Conduct comprehensive needs assessments for individuals and families requiring social care support.
• Develop, implement, and monitor personalized support plans, ensuring they meet agreed outcomes.
• Provide information, advice, and guidance to clients on available services and entitlements.
• Liaise with partner agencies, health services, and community resources to facilitate integrated care.
• Maintain accurate and confidential records of assessments, interventions, and outcomes.
• Regularly review and evaluate the effectiveness of support plans, making adjustments as necessary.

Requirements

• A qualification in social work, social care, or a related discipline (e.g., NVQ Level 3/4, Diploma in Social Care).
• Previous experience in a social care assessment or case management role.
• Strong understanding of current social care legislation, policies, and best practices.
• Excellent communication, negotiation, and interpersonal skills.
• Ability to work effectively under pressure and manage a diverse caseload.
• Proficiency in using IT systems for record-keeping and case management.
