Social Care Officer

About the Role

Our client is recruiting for a proactive Social Care Officer to contribute to their community services team in Coventry . This role is central to assessing needs, developing support plans, and coordinating services for vulnerable individuals and families. You will work collaboratively with a multidisciplinary team to ensure clients receive appropriate and timely support, promoting their independence and well-being. This hybrid role offers a balance of on-site engagement within the Coventry community and remote administrative duties, allowing for flexibility while maintaining essential face-to-face client interaction.

Key Responsibilities

• Conduct comprehensive needs assessments for individuals and families requiring social care support.
• Develop, implement, and review personalised support plans in conjunction with clients and stakeholders.
• Liaise with various agencies, including health services, housing associations, and educational institutions.
• Provide guidance and support to clients navigating social care systems and accessing community resources.
• Maintain accurate and confidential records of client interactions, assessments, and progress.
• Advocate for clients' needs and ensure their rights and safety are upheld.

Requirements

• Proven experience in a social care or related field.
• Knowledge of relevant legislation and policies governing social care in the UK.
• Strong assessment, planning, and communication skills.
• Ability to work autonomously and as part of a team, both remotely and in the community.
• Proficiency in using IT systems for record-keeping and communication.
