About the Role

Our client is seeking a compassionate and organised Social Care Officer to join their team based in Birmingham, West Midlands, UK . This role involves working with individuals and families to assess needs, provide support, and facilitate access to relevant services. You will play a crucial role in empowering individuals to achieve greater independence and improve their quality of life, working closely with local authorities and community partners to deliver tailored care packages. The position offers a hybrid working model, blending essential fieldwork with the flexibility of remote administrative tasks.

Key Responsibilities

• Conduct comprehensive assessments of client needs, both in-person and remotely.
• Develop, implement, and review person-centred support plans in collaboration with clients.
• Provide ongoing support, advice, and advocacy to individuals and their families.
• Liaise effectively with statutory and voluntary sector agencies to coordinate services.
• Maintain accurate case records and prepare reports for management and external bodies.
• Respond to safeguarding concerns and follow established procedures to ensure client safety.

Requirements

• Experience in a social care or related field, with a good understanding of relevant legislation and policies.
• Strong assessment, planning, and review skills.
• Excellent communication, interpersonal, and negotiation abilities.
• Ability to work effectively both independently and as part of a team, managing a hybrid schedule.
• Proficiency in using case management systems and standard office software.
