Social Care Officer

About the Role

Our client is seeking a proactive and skilled Social Care Officer to join their team in Coventry . Key Responsibilities

• Conduct comprehensive assessments of individuals' needs, identifying appropriate support strategies and resources.
• Develop, implement, and review personalized support plans in collaboration with service users and their families.
• Liaise effectively with external agencies, healthcare professionals, and community groups to coordinate services and support networks.
• Provide emotional support and practical assistance to individuals, helping them to overcome challenges and achieve their personal goals.
• Monitor client progress, document care activities meticulously, and report on outcomes and any changes in needs.
• Advocate for the rights and needs of service users, ensuring they receive the best possible care and support.

Requirements

• Proven experience in a social care or support role, with a good understanding of relevant legislation and best practices.
• NVQ/QCF Level 3 or equivalent in Health and Social Care is highly preferred.
• Excellent assessment, planning, and report-writing skills.
• Strong communication and interpersonal skills, with the ability to engage effectively with a diverse range of individuals.
• A valid UK driving license and access to a vehicle may be advantageous for community-based work.
• Commitment to maintaining confidentiality and upholding the highest standards of professional conduct.
