Stroke Recovery Support Coordinator

Carer Support Wiltshire is seeking an innovative, compassionate Stroke Support Coordinator to help stroke survivors and families adjust to life after stroke. The role is home-based across Herefordshire and Worcestershire with extensive travel and part-time hours.

Reporting to the Stroke Service Delivery Coach, you’ll provide personalised information, set goals with clients, and collaborate with NHS colleagues to connect people with the right support.
