About the Role
Our client is seeking a proactive and organized Support Coordinator to join their expanding disability services team in Bristol, South West England, UK . This role is crucial for empowering individuals with disabilities to live fulfilling and independent lives by coordinating essential support services tailored to their unique needs. You will work closely with clients, their families, and a network of service providers to ensure seamless delivery of support, fostering choice, dignity, and inclusion within the Bristol community and beyond, championing person-centred approaches in all aspects of service delivery.
Key Responsibilities
- Develop, implement, and review individualized support plans in consultation with clients and their support networks.
- Coordinate and manage a range of services, including personal care, domestic assistance, and community participation.
- Liaise with external agencies, healthcare professionals, and specialist services to ensure integrated and holistic support.
- Monitor the effectiveness of support arrangements, making adjustments as necessary to meet evolving needs.
- Provide information, advice, and advocacy to clients and their families regarding their rights and available services.
- Maintain accurate and confidential client records and reports, ensuring compliance with organizational and legislative standards.
Requirements
- Experience in disability support, case management, or a related field.
- Understanding of disability issues, person-centred planning, and relevant legislation.
