About the Role
Our client is seeking a dedicated and experienced Support Coordinator specializing in learning disabilities to join their fully remote team, serving the Stoke-on-Trent area. In this critical role, you will remotely manage and coordinate comprehensive support plans for individuals with learning disabilities, ensuring they receive the highest quality of care and opportunities to achieve their personal goals. You will liaise with clients, families, support staff, and external agencies to facilitate seamless service delivery and advocate for the needs of the individuals you support. Key Responsibilities
- Develop, implement, and review personalised support plans tailored to the specific needs and aspirations of individuals with learning disabilities.
- Coordinate and schedule a range of support services, including daily living assistance, community access, and therapeutic interventions.
- Act as a primary point of contact for clients, their families, and support staff, providing guidance and resolving concerns.
- Liaise effectively with healthcare professionals, social services, and other relevant agencies to ensure integrated and holistic care.
- Monitor the effectiveness of support services, gathering feedback and making necessary adjustments to care plans.
- Maintain accurate and up-to-date electronic records of all client interactions, service provision, and progress reports.
Requirements
- Proven experience in supporting individuals with learning disabilities, ideally in a coordination or case management role.
- In-depth understanding of person-centred planning, positive behaviour support, and advocacy principles.
- Excellent communication, interpersonal, and organisational skills.
- Ability to manage a caseload effectively and prioritise tasks in a remote environment.
- Proficiency in using digital communication platforms and record-keeping systems.
