Support Coordinator - Learning Disabilities

About the Role

Our client is seeking an experienced and dedicated Support Coordinator with a focus on Learning Disabilities to join their team in Plymouth . You will be instrumental in empowering individuals with learning disabilities to lead fulfilling and independent lives by developing and overseeing personalised support plans. This role requires a compassionate individual with a strong understanding of person-centred planning and advocacy, working closely with clients, their families, and a network of support services. Your role will involve coordinating practical assistance, promoting community inclusion, and ensuring clients' rights and choices are upheld.

Key Responsibilities

• Conduct comprehensive assessments of individuals' needs and aspirations related to learning disabilities.
• Develop, implement, and regularly review person-centred support plans in collaboration with clients and stakeholders.
• Coordinate a range of services and support activities to meet identified needs, including daily living, education, and employment.
• Act as a key advocate for clients, ensuring their voices are heard and their choices respected.
• Build and maintain positive working relationships with families, support workers, health professionals, and community organisations.
• Monitor the effectiveness of support plans and make necessary adjustments, documenting all progress accurately.

Requirements

• Previous experience in supporting individuals with learning disabilities.
