About the Role
Our client is seeking a proactive and organised Support Coordinator to manage and deliver comprehensive support services to individuals within the Oxford area. This role is central to ensuring that clients receive personalised care that meets their specific needs, fostering independence and enhancing their quality of life. You will work closely with clients, their families, and a team of support professionals to develop and implement effective support plans. Key Responsibilities
- Develop, implement, and regularly review individualised support plans in collaboration with clients and their support networks.
- Coordinate the provision of care and support services, ensuring timely and effective delivery according to agreed plans.
- Act as a primary point of contact for clients, families, and external agencies, addressing queries and facilitating communication.
- Liaise with healthcare professionals and other service providers to ensure a holistic and integrated approach to care.
- Monitor client progress and well-being, making adjustments to support plans as necessary and reporting on outcomes.
- Maintain accurate and up-to-date client records and documentation in compliance with organisational policies and data protection regulations.
Requirements
- Previous experience in a support coordination, care management, or similar role within the social care sector.
- Strong understanding of person-centred care principles, safeguarding procedures, and relevant legislation.
- Excellent organisational, planning, and time management skills.
- Effective communication and interpersonal skills, with the ability to build strong relationships.
- Proficiency in using care management software and standard office applications.
- A proactive approach to problem-solving and a commitment to achieving positive client outcomes.
