About the Role
Our client is seeking an organised and proactive Support Coordinator to join their organisation, primarily based in Leeds , with a strong remote working component. This role is central to ensuring clients receive timely and appropriate support services, acting as a key point of contact and facilitating seamless care delivery. You will be instrumental in assessing needs, developing support plans, and coordinating with external agencies and internal teams. Key Responsibilities
- Conduct comprehensive needs assessments for individuals requiring support services.
- Develop, implement, and monitor personalized support plans in collaboration with clients and stakeholders.
- Coordinate referrals and liaise with health and social care professionals, external agencies, and service providers.
- Maintain accurate and detailed client records and case notes.
- Provide guidance and support to clients and their families navigating care systems.
- Evaluate the effectiveness of support interventions and adjust plans as necessary.
Requirements
- Proven experience in a similar role within social care, health, or community services.
- Strong understanding of care pathways, funding streams, and relevant legislation.
- Excellent assessment, planning, and coordination skills.
- Proficiency in IT, including case management systems and standard office software.
- Strong interpersonal and communication abilities, with a client-centred approach.
- Ability to manage a caseload effectively and work autonomously.
