Key Responsibilities

• Conduct comprehensive assessments of individuals' support needs and capabilities.
• Develop, implement, and regularly review personalised support plans in collaboration with clients and their families.
• Coordinate and arrange services from a range of external providers, ensuring quality and reliability.
• Act as a key point of contact for clients, families, and support professionals, facilitating clear communication.
• Monitor client progress and satisfaction, making adjustments to support plans as necessary.
• Maintain accurate and confidential records of assessments, plans, and service delivery.

Requirements

• Experience in social care, community support, or a related field.
• Excellent assessment, planning, and organisational skills.
• Strong communication and interpersonal abilities, with the capacity to build rapport.
• Knowledge of available support services and resources within the local area.
• Ability to work autonomously and as part of a team, managing a caseload effectively.
• A relevant qualification in health and social care is desirable.
