Support Worker

About the Role

Our client is actively recruiting for a compassionate and proactive Support Worker to join their dynamic team based in Newcastle upon Tyne . This role focuses on empowering individuals with various needs, including learning disabilities and mental health challenges, to achieve greater independence and enhance their quality of life. You will be integral to providing person-centred support that respects the dignity and choices of each client. Key Responsibilities

• Deliver personalised support plans tailored to the individual needs and goals of each client.
• Assist clients with daily routines, including personal care, domestic tasks, and accessing community resources.
• Promote client independence and choice, encouraging them to participate actively in decision-making processes.
• Observe, report, and record client progress and any changes in their condition or behaviour.
• Liaise effectively with families, carers, and other professionals to ensure a coordinated approach to care.
• Facilitate engagement in meaningful activities, hobbies, and social opportunities to combat isolation.

Requirements

• Previous experience in a support role, particularly with adults with complex needs, is highly valued.
• Demonstrated understanding of person-centred planning and outcome-focused support.
• Excellent communication, listening, and empathy skills.
• Ability to maintain accurate documentation and adhere to organisational policies.
• A commitment to promoting equality, diversity, and inclusion in all aspects of care.
• Willingness to work flexibly, including evenings, weekends, and sleep-in shifts as required.
