Key Responsibilities

• Provide personalised support to individuals, assisting with personal care, domestic tasks, and community integration.
• Encourage and support service users to make their own choices and decisions about their lives and care.
• Implement and follow individualised support plans, ensuring all needs are met effectively and safely.
• Observe and report on the well-being and progress of service users, maintaining accurate and confidential records.
• Promote social inclusion by supporting individuals to participate in local activities, appointments, and social events.
• Work collaboratively with families, other support staff, and external agencies to ensure a cohesive care approach.

Requirements

• Experience in a support role within the community care sector is advantageous.
• Strong understanding of person-centred care principles and how to apply them.
• Excellent communication and listening skills, with a patient and understanding demeanour.
• Ability to work independently and as part of a team, demonstrating initiative and reliability.
• Willingness to undergo background checks and relevant training, including mandatory qualifications.
• Must be able to work flexibly according to service needs, including evenings and weekends.
