About the Role

Our client is looking for an organized and empathetic Care Coordinator to manage community-based support services in Cardiff . Key Responsibilities

• Develop and manage individual care plans, ensuring they meet the assessed needs of clients.

• Act as the primary point of contact for clients, their families, and care providers, addressing queries and concerns promptly.
• Liaise with external agencies, including healthcare professionals and local authorities, to facilitate integrated care.
• Monitor the quality of care provided, conducting regular reviews and implementing improvements as needed.
• Maintain accurate client records and ensure compliance with all relevant regulations and company policies.

Requirements

• Previous experience in care coordination, domiciliary care management, or a similar role.
• Strong understanding of care planning, risk assessment, and safeguarding principles.
• Excellent organisational, time management, and problem-solving skills.
• Proficient in using care management software and standard office applications.
