About the Role

Our client is looking for a motivated and proactive Community Outreach Worker to join their team in Bradford . This role is focused on engaging with diverse communities, identifying individuals in need of support, and connecting them with vital local services. You will play a key part in building trust and relationships within the community, acting as a bridge between residents and available resources. Key Responsibilities

• Actively engage with individuals and groups within the community to build rapport and identify support needs.
• Provide information, advice, and signposting to relevant services, including health, housing, and employment.

• Develop and maintain relationships with local organizations, community leaders, and service providers.
• Assist individuals in accessing services and overcoming barriers to participation.
• Collect data and feedback to inform service development and outreach strategies.

Requirements

• Experience in a community-facing role, social care, or related field.
• Excellent interpersonal and communication skills, with the ability to engage diverse groups.
• Understanding of the challenges faced by various communities within Bradford .
