About the Role
Our client is seeking a motivated and proactive Community Outreach Worker to join their fully remote team, dedicated to serving the Derby community. In this role, you will be instrumental in connecting vulnerable individuals and families with essential support services and resources, bridging the gap between need and provision. Working remotely, you will engage with clients through various digital channels, assess their situations, and guide them towards appropriate assistance, fostering greater social inclusion and well-being. Key Responsibilities
- Identify and engage with individuals and families in need of support within the community.
- Conduct needs assessments via phone and video calls to understand client circumstances.
- Provide information and guidance on available social services, health resources, and community programs.
- Facilitate client access to necessary services, assisting with applications and appointments.
- Maintain accurate and confidential client records and track progress.
- Collaborate with local agencies, charities, and support groups to expand service provision.
Requirements
- Previous experience in a social care, community development, or outreach role.
- Strong knowledge of local services and support networks within the Derby area.
- Excellent communication, interpersonal, and active listening skills for remote engagement.
- Proficiency in using digital communication platforms and standard office software.
- Ability to work independently, manage a caseload, and demonstrate initiative.
