About the Role
Our client is seeking a proactive and empathetic Community Support Officer to join their fully remote team, serving individuals across the Birmingham area and beyond. This role is crucial for providing essential support, guidance, and advocacy to individuals and families facing various social and economic challenges. Working from the comfort of your own home, you will leverage digital tools and communication platforms to connect with clients, assess their needs, and facilitate access to vital community resources and services, fostering independence and improving overall well-being.
Key Responsibilities
- Conduct needs assessments and develop personalized support plans for clients via phone, video calls, and online platforms.
- Provide information, advice, and advocacy on a range of issues including housing, benefits, and social inclusion.
- Facilitate referrals to specialist services and other relevant support agencies, maintaining strong working relationships.
- Monitor client progress and well-being, adjusting support plans as required and providing consistent follow-up.
- Utilize case management software to accurately record client interactions, progress, and outcomes.
- Organise and facilitate online support groups and community engagement activities to foster connection and reduce isolation.
Requirements
- Previous experience in a social care, community work, or support role is essential.
- Excellent communication and active listening skills, with the ability to empathise and connect with diverse client groups remotely.
- Proficiency in using digital communication tools, video conferencing software, and online case management systems.
- Strong organisational skills and the ability to manage a caseload independently and effectively.
- A genuine passion for supporting vulnerable individuals and promoting social inclusion.
