Our client is seeking a proactive and organised Community Support Officer to join their dedicated team in Coventry . This role is crucial in connecting vulnerable individuals with the resources and support networks they need to thrive. You will be instrumental in developing and implementing support plans, coordinating services, and empowering clients to achieve their personal goals. Key Responsibilities
- Conduct assessments of client needs and develop tailored support plans in collaboration with individuals and their families.
- Liaise with external agencies, charities, and local authorities to secure appropriate services and resources for clients.
- Provide guidance and advocacy for clients, helping them navigate complex systems and access benefits.
- Organise and facilitate group activities and workshops to promote social inclusion and skill development.
- Monitor client progress, review support plans regularly, and adapt strategies as needs change.
- Maintain accurate case notes and administrative records in accordance with organisational policies and data protection regulations.
Requirements
- Proven experience in a social care , case management, or community outreach role.
- Excellent understanding of welfare rights, housing support, and mental health services.
- Strong negotiation, problem-solving, and organisational skills.
- Ability to build rapport and establish trusting relationships with a diverse client base.
- Proficiency in using IT systems for record-keeping and communication.
- A valid UK driving license and access to a vehicle for travel within the Coventry area may be required.
