Key Responsibilities
- Conduct needs assessments and develop tailored support plans for individuals in the community.

- Maintain accurate client records and prepare reports on progress and outcomes.
- Collaborate with local authorities, charities, and other support networks to ensure comprehensive client care.
- Respond to client needs via phone, email, and scheduled in-person visits.
Requirements
- Experience in a social care, community support, or similar role.
- Excellent understanding of the challenges faced by vulnerable individuals.
- Strong organisational and time-management skills, with the ability to manage a caseload.
- Effective communication and interpersonal skills, with a compassionate approach.
- Ability to work independently and as part of a team, adapting to hybrid working arrangements.
- Proficiency in basic IT skills for record-keeping and communication.
