Family Support Outreach Worker

About the Role

Our client is seeking a motivated and empathetic Family Support Outreach Worker to join their team in Bristol . This hybrid role involves working directly with families in their homes and community settings, as well as undertaking remote case management and coordination. The primary focus is to provide practical support, guidance, and advocacy to families experiencing difficulties, helping them to improve their well-being and resilience. You will play a key role in empowering families to access resources, build positive relationships, and achieve sustainable change. Key Responsibilities

• Conduct comprehensive family needs assessments to identify strengths, challenges, and support requirements.
• Develop and implement tailored family support plans in collaboration with families and relevant professionals.
• Provide practical support and guidance to families on issues such as parenting skills, child development, household management, and accessing services.
• Act as a liaison between families and other agencies, including schools, health services, and social care.
• Deliver targeted interventions to address specific family needs, such as conflict resolution or emotional support.
• Maintain accurate, confidential records of assessments, interventions, and progress, utilising digital case management systems.

Requirements

• Experience working with families or vulnerable individuals in a social care, community, or related field.
• A strong understanding of child development, parenting challenges, and common family stressors.
• Excellent communication, interpersonal, and negotiation skills, with the ability to build rapport and trust with families.
• Proficiency in IT, including case management software and standard office applications, essential for hybrid working.
• A valid driving licence and access to a car for home visits across the Bristol area are required.
• Relevant qualifications such as a Diploma in Health and Social Care, Social Work, or equivalent are advantageous.
