Our client is seeking a compassionate and dedicated Mental Health Support Worker to join their Community & Social Care team in Derby . This role is crucial in providing support to individuals experiencing mental health challenges, empowering them to manage their conditions and improve their quality of life. You will offer emotional support, assist with daily living tasks, and help clients access community resources and therapeutic services. This hybrid role allows for a balance between direct client support and remote administrative or coordination tasks, offering flexibility and impact.
Key Responsibilities
- Provide emotional and practical support to individuals with a range of mental health conditions.
- Assist clients in developing and implementing personalized recovery and support plans.
- Encourage and support clients to engage in meaningful activities and community integration.
- Monitor clients' well-being, reporting any concerns or changes to the appropriate healthcare professionals.
- Help clients develop coping strategies and self-management skills for their mental health.
- Liaise with family members, carers, and external mental health services to ensure coordinated care.
Requirements
