About the Role

Our client is looking for a compassionate and efficient Remote Care Navigator to guide individuals and families through the complexities of accessing social and healthcare services. Operating entirely from home, you will be the first point of contact for clients seeking support, assessing their needs and connecting them with appropriate resources and providers. This role is fundamental to ensuring equitable access to care and support within the community, requiring excellent communication skills and a deep understanding of available services. You will be instrumental in empowering clients to make informed decisions about their care pathways.

Key Responsibilities

• Conduct initial needs assessments via phone and video conferencing to understand client requirements.
• Provide information and guidance on available social care, health, and community support services.
• Facilitate referrals to relevant agencies, charities, and healthcare professionals.
• Develop and maintain relationships with service providers to ensure up-to-date knowledge of offerings.
• Track client progress and follow up to ensure satisfaction and successful integration of services.
• Maintain accurate and confidential client records on the organisation's digital platform.

Requirements

• Experience in a social work, healthcare support, or community outreach role.
• Knowledge of local authority services and third-sector organisations.
• Excellent listening, communication, and interpersonal skills, suitable for remote interaction.
• Strong organisational skills and the ability to manage a varied caseload.
• Proficiency in using digital tools for communication and record-keeping.
