About the Role

Our client is seeking a proactive and organised Remote Care Navigator to support individuals within the Portsmouth area, operating entirely from a home-based position. Key Responsibilities

• Serve as the main point of contact for assigned clients, providing support and information.
• Assist clients in scheduling medical appointments and accessing necessary services.
• Help clients understand their treatment plans and medication regimens.
• Facilitate communication between clients, healthcare providers, and social services.
• Maintain detailed and accurate client records in the electronic health system.
• Identify barriers to care and work to resolve them for the client.

Requirements

• Previous experience in a healthcare support, administration, or patient navigation role.
• Excellent communication, empathy, and interpersonal skills for remote interaction.
• Proficiency in using digital communication platforms and Electronic Health Records (EHR).
• Strong organisational skills and the ability to manage multiple client cases effectively.
