Our client is seeking a meticulous and organised Remote Care Planner to join their virtual operations team. This position is essential for developing comprehensive and personalised care plans for individuals requiring support, ensuring their needs are met effectively and efficiently from a remote location. You will collaborate closely with clients, families, and healthcare professionals to create bespoke support strategies, optimising service delivery and client outcomes. Key Responsibilities
- Develop detailed, person-centred care plans based on individual assessments and needs.
- Liaise effectively with clients, their families, support networks, and external agencies.

Requirements
