About the Role
Our client is looking for a proactive and organized Remote Support Coordinator to manage and facilitate community support services from a remote setting. In this pivotal role, you will be the central point of contact, coordinating care plans, liaising with support workers and clients, and ensuring the smooth delivery of services across various locations, including Portsmouth and surrounding areas, though your work will be performed remotely. You will utilize digital tools and communication platforms to maintain high standards of service and provide essential administrative and logistical support to the field teams, ensuring clients receive timely and effective care.
Key Responsibilities
- Coordinate and schedule support worker visits and client appointments using digital scheduling software.
- Act as the primary point of contact for clients, their families, and support workers, addressing queries and concerns promptly.
- Maintain accurate and up-to-date client records and service logs within the CRM system.
- Liaise with external agencies and healthcare professionals to facilitate seamless care transitions and information sharing.
- Monitor service delivery, identify potential issues, and implement solutions to ensure continuity and quality of care.
- Generate reports on service utilization, client feedback, and team performance for management review.
Requirements
- Proven experience in a coordination, administrative, or customer support role, preferably within the community care sector.
- Excellent organizational and time management skills, with the ability to multitask effectively.
- Strong proficiency with IT systems, including CRM software, scheduling tools, and Microsoft Office Suite.
- Exceptional communication and interpersonal skills, with the ability to build rapport remotely.
- Ability to work independently and manage workload autonomously in a remote environment.
