Support Coordinator - Elderly Care

About the Role

Our client is seeking a compassionate Support Coordinator to oversee services for elderly individuals in Edinburgh . This role is based within the community and requires regular interaction with clients, their families, and healthcare professionals to ensure the highest standards of care and support are met. You will be responsible for developing personalised care plans, coordinating services, managing a team of care staff, and ensuring regulatory compliance. The focus is on enabling older adults to live with dignity, independence, and quality of life within their own homes and communities throughout the Scotland capital.

Key Responsibilities

• Assess the care and support needs of elderly clients, developing and implementing comprehensive, person-centred care plans.
• Coordinate the delivery of a range of support services, including personal care, domestic assistance, and social activities.
• Manage and supervise a team of care assistants, including scheduling, training, and performance monitoring.
• Ensure all care provided meets the highest standards and complies with relevant health and safety regulations and CQC guidelines.
• Liaise with families, doctors, social workers, and other healthcare professionals to ensure a holistic approach to client care.
• Conduct regular reviews of care plans and client satisfaction, making necessary adjustments to service delivery in Edinburgh .

Requirements

• Proven experience in a care coordination or supervisory role, preferably within elderly care or domiciliary services.
• A strong understanding of the needs and challenges faced by older adults, including common health conditions and safeguarding issues.
• Excellent communication, organisational, and leadership skills, with the ability to manage a team effectively.
• Proficiency in using care management software and standard office applications.
• A valid driving licence and willingness to travel within the Edinburgh area is essential for this role.
• Relevant qualifications such as an SVQ in Health and Social Care (Level 3 or above) are highly desirable.
