About the Role
Our client is seeking a dedicated and organised Support Coordinator with a specialization in supporting individuals with learning disabilities, based in Reading . This hybrid role requires a proactive individual who can manage care plans, coordinate support services, and liaise with families and professionals to ensure the highest quality of care. While some community visits and team meetings will require your presence in the Reading area, you will also benefit from remote working flexibility for administrative tasks, planning, and communication. You will play a key role in empowering individuals to live fulfilling lives within their communities.
Key Responsibilities
- Conduct comprehensive assessments of individuals' support needs, developing and implementing person-centred care plans.
- Coordinate the provision of support services, including scheduling staff, managing rotas, and ensuring appropriate resources are available.
- Regularly review care plans with individuals, families, and support teams, making necessary adjustments.
- Act as a key liaison between individuals, their families, external health and social care professionals, and support staff.
- Ensure compliance with all relevant regulations, policies, and best practice guidelines.
- Maintain accurate and up-to-date records and documentation for all clients under your care.
Requirements
- Relevant qualification in Health and Social Care, such as NVQ Level 3 or above.
- Proven experience in coordinating care or support services, particularly for individuals with learning disabilities.
- Strong understanding of person-centred planning, safeguarding, and relevant legislation.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to manage time effectively and prioritise tasks in a busy environment.
- Full UK driving license and access to a vehicle for local travel.
