About the Role
Our client is seeking a compassionate and organised Support Coordinator to join their dedicated team in Portsmouth , focusing on individuals with learning disabilities. This role is pivotal in ensuring clients receive tailored support that promotes independence, choice, and inclusion within their community. You will be responsible for coordinating a range of services, building positive relationships with clients and their families, and working collaboratively with external agencies. This hybrid role allows for a mix of client visits within Portsmouth and administrative tasks conducted remotely or from the office, offering a dynamic working environment.
Key Responsibilities
- Develop and implement personalised support plans based on individual needs and aspirations.
- Coordinate and arrange services from various providers, ensuring seamless integration of care.
- Act as a primary point of contact for clients, their families, and support networks.
- Conduct regular reviews of support plans, making necessary adjustments to meet evolving needs.
- Advocate for clients' rights and choices, ensuring their voices are heard.
- Maintain accurate and comprehensive client records and reports.
Requirements
- Experience in social care, particularly supporting individuals with learning disabilities.
- Understanding of principles of enablement, independence, and person-centred planning.
- Excellent communication, interpersonal, and organisational skills.
- Ability to work collaboratively with clients, families, and a multidisciplinary team.
- Proficiency in IT skills, including case management systems.
- Relevant qualification (e.g., NVQ/QCF Level 3 or 4 in Health and Social Care) is desirable.
