About the Role
Our client is seeking a proactive and compassionate Support Coordinator to manage and deliver mental health support services in the Nottingham area. This role is integral to ensuring individuals experiencing mental health challenges receive timely, appropriate, and person-centred support. You will work collaboratively with clients, their families, and a network of healthcare and community professionals to develop tailored support plans and facilitate access to necessary resources, promoting recovery and well-being within a supportive framework.
Key Responsibilities
- Conduct thorough assessments of clients' mental health needs and develop comprehensive, person-centred support plans.
- Coordinate the delivery of support services, liaising with healthcare professionals, social services, and community groups.
- Monitor client progress, regularly review support plans, and make adjustments as needed to ensure effectiveness.
- Provide direct support and interventions where appropriate, empowering clients to manage their conditions and achieve their goals.
- Maintain accurate and confidential case records, ensuring compliance with data protection and professional standards.
- Facilitate access to therapeutic services, employment opportunities, and social activities to promote recovery and integration.
Requirements
- Experience in a mental health setting, ideally in a coordination or support role.
- Strong understanding of mental health conditions, recovery models, and support strategies.
- Excellent communication, organisational, and problem-solving skills.
- Ability to work effectively with a range of stakeholders, including clients, families, and professionals.
- Relevant qualification in mental health or social care is highly desirable.
