About the Role
Our client is seeking a proactive and empathetic Community Support Officer to join their dynamic team in Leeds . This role focuses on providing essential support and guidance to individuals within the community who may be facing various challenges, including mental health issues, homelessness, or social isolation. You will work to empower individuals to access resources, develop life skills, and improve their overall quality of life. This position offers the flexibility of remote working combined with essential in-person outreach, allowing for significant impact across the local community.
Key Responsibilities
- Provide one-to-one support to clients, helping them to achieve their personal goals and improve their independence.
- Assist clients in accessing community resources, such as housing, employment, healthcare, and social services.
- Develop and implement tailored support plans based on individual client needs and aspirations.
- Facilitate group activities and workshops to promote social inclusion and skill development.
- Maintain accurate and confidential client records, documenting progress and interventions.
- Liaise with external agencies and stakeholders to build and maintain effective support networks.
Requirements
- Previous experience in a Community Support or related role.
- Demonstrable understanding of issues affecting vulnerable adults, such as mental health, addiction, and homelessness.
- Excellent listening, communication, and interpersonal skills.
- Ability to build rapport and trust with individuals from diverse backgrounds.
- Proficiency in record-keeping and case management.
- A valid driving license and access to a vehicle for outreach work may be required.
