About the Role
Our client is looking for a dedicated Support Coordinator to join their team in Edinburgh , focusing on individuals with learning disabilities. This role is crucial in empowering clients to lead fulfilling and independent lives by coordinating bespoke support services. You will work collaboratively with clients, their families, and a network of service providers, ensuring that support is person-centered, effective, and responsive to evolving needs. Operating on a hybrid basis, this position offers a blend of community-based interaction and remote administrative duties, providing a flexible and impactful career within the social care sector in Edinburgh .
Key Responsibilities
- Conduct thorough assessments of individuals' support needs and aspirations related to learning disabilities.
- Develop, implement, and regularly review person-centered support plans in close collaboration with clients and stakeholders.
- Coordinate and arrange a range of services, including personal care, community access, and therapeutic interventions.
- Liaise effectively with families, carers, health professionals, and other external agencies to ensure integrated support.
- Monitor the quality and effectiveness of support services, making adjustments as necessary.
- Maintain accurate and confidential client records and manage administrative tasks efficiently.
Requirements
- Proven experience in supporting individuals with learning disabilities or other complex needs.
- Strong understanding of principles of enablement, independence, and person-centered planning.
- Excellent organizational, planning, and time-management skills.
- Effective communication and interpersonal skills, with the ability to build rapport and trust.
- Proficiency in using IT systems for record-keeping and communication; hybrid working experience is advantageous.
